6/29/2023 EnchantaVaughn Approved AM) Upon arrival, Client was observed settling into summer camp unpacking and finishing his breakfast. Client was in a disappointed mood, as evidenced by his frown and teary eyes due to not being chosen to be today’s line leader. Client was upset but still remained cooperative during morning activity. Client was seen interacting well with his peers and engaging in his group. Client displayed mannerisms in various settings, returning to class without incident. Client was witnessed listening and following directions and did not have to be corrected for his behaviors. Client was active in his work and completed all assignments in a timely manner, as reported. (PM) Client communicates with his teacher when he needs assistance and is supported for complying to teacher instructions. Client is civilized and well behaved throughout the day, staying on task and paying attention in class. Client transitions to lunch and end of the day activities with a positive attitude. Client is monitored displaying appropriate social skills and keeping his hands to himself. Client engaged in group discussion, making insightful comments, and responding well to feedback. Client is polite to peers and staff and is praised for keeping a respectful attitude. (EOD) Client continues to treat others with respect and is pleasant toward mental health professionals. Client exhibits self control and moderate self esteem when faced with challenging duties (e.g. answering hard questions and solving difficult problems on his own). Client is observed displaying good behaviors as he prepared for dismissal, packing his book bag and waiting for his bus, ending the day meeting objectives 1, 2, and 3.(AM) QMHP welcomed client to school and provided support and encouragement to ensure a good start to his day and to be on track with achieving his goals and objectives. QMHP monitored client individualized behaviors in collaboration with his teacher and other mental health professional. QMHP encouraged client to make good choices and to utilize self calming techniques (i.e. counting and deep breathing) and processed with him that this behavior is acceptable and discussed actions and consequences. QMHP challenged client to continue displaying a positive attitude while employing positive reinforcement when client is able to achieve his goals and objectives. QMHP provided praise when the client continued to display positive behaviors throughout the day, remaining on task and was very respectful to peers and authority. QMHP isolated client from distractions to decrease disruptive behaviors and to reduce excessive talking in class. QMHP explored the client’s capability to cope with his stressors. QMHP brainstormed how to resolve conflict without engaging in aggressive behaviors.(AM) Client greeted QMHP with a hug. Client verbalized positive feelings about being in school and was excited to see QMHP. Client demonstrated understanding after reviewing goals and objectives. Client displayed high level of participation in class, lunch, and gym settings. (PM) Client responded in a positive manner accepting the positive reinforcement that the QMHP provided by saying "thank you". The client has displayed minor frustration over age-related declining abilities and decided to look at the bright side of things. The client has continued to share his feelings and has been assisted in identifying causes for them. Client explains that he struggles with certain activities in class and wishes to be taught on a one on one level. (EOD) Client was involved in effective learning group and showed interest to his peers. Client was in a cheerful mood. Client nodded his head and said I understand. Client realized he was capable of being successful and started working harder.
